Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here the direction is a little different from what you might expect. Both letrozole and ribociclib (the two medicines in Kisqali Femara Co-Pack) can act as mild blockers of a liver enzyme called CYP3A4. Evodia happens to be cleared by that same enzyme. So in theory, your cancer medicines could slow down how your body breaks down evodia, letting evodia's compounds build up higher and linger longer.

Important to keep in mind: this is based only on animal and test-tube research, not people, so it's a theoretical concern. We don't have good safety data on evodia alongside these drugs anyway. Ribociclib in particular needs careful handling, so before adding any herbal supplement, please run it by your oncologist or pharmacist first.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the short version on combining Evodia with your Kisqali and Femara co-pack. The ribociclib part of this combo is already known to affect your heart's electrical timing, something doctors call QT prolongation. That's exactly why your team usually checks an EKG before and during treatment. Evodia, in lab and animal studies, has also nudged the heart's rhythm in that same direction.

So the concern is an additive one: stacking two things that stretch the QT interval could, in theory, raise the risk of a dangerous irregular heartbeat. I want to be honest that this is based only on test-tube and animal data, not human cases, so it's not a reason to panic. Still, given that heart rhythm is already a known issue with ribociclib, please check with your oncologist or pharmacist before adding Evodia.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

This combo pill pairs letrozole with ribociclib for breast cancer treatment. Ribociclib in particular leans on a liver enzyme called CYP3A4 to be cleared from your body. Lab studies suggest Evodia may slow that same enzyme down. In theory, that could let ribociclib build up a bit higher than expected, which might mean more of its side effects like low blood counts, nausea, fatigue, or changes in your heart rhythm (QT prolongation).

I want to be honest about the evidence here: this is only seen in test tubes, not in real people, so we don't truly know if it matters. Still, because cancer therapy is serious and dosing is precise, please don't add Evodia without first checking with your oncologist or pharmacist.
