Ketoconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for ketoconazole: your body partly clears it using a liver enzyme called CYP3A4. In lab dish testing only, False Unicorn appeared to slow that same enzyme down. If that held true in people, ketoconazole might break down a little slower and build up in your system, which could mean a bit more of its usual effects, like nausea, stomach upset, or in rare cases liver irritation.

I want to be honest though: this comes from test-tube data, not real human studies, so it's mostly theoretical and far from proven. Interestingly, ketoconazole itself is a strong CYP3A4 blocker, so the two could nudge each other. I wouldn't lose sleep, but please check with your pharmacist or doctor before pairing them.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
