Ketoconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for ketoconazole. Your body clears this antifungal partly through a liver enzyme called CYP3A4. Some lab research hints that lentinan might speed that enzyme up. If that happened in real life, your body could break down ketoconazole a little faster, which might leave less of it in your system to fight the fungal infection. In plain terms, the worry is a weaker antifungal effect, not more side effects.

I want to be honest about how thin this evidence is. This has only shown up in test tube or animal studies, never in actual people, so it may not matter at all. Still, if you're treating a real infection, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
