Esomeprazole Magnesium falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the gist for Nexium (esomeprazole). Your body uses an enzyme called CYP2C19 to break this medicine down. Lab studies hint that lentinan might speed that enzyme up a bit, which in theory could clear esomeprazole faster and leave less of it working in your system. If that happened, you might notice your acid control slipping, so think more heartburn, that burning feeling in your chest, or sour reflux creeping back.

I want to be honest about the strength here: this is only test-tube and animal research, and it has never been reported in real people. So it's more of a theoretical heads-up than something to lose sleep over. If you take both, just keep an eye on whether your symptoms return, and check in with your pharmacist or doctor before changing anything.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with Nexium (esomeprazole) and lentinan. Your body clears esomeprazole partly through a liver enzyme called CYP3A4, though it relies even more on a related enzyme (CYP2C19). Lab studies hint that lentinan may speed up CYP3A4, which could theoretically clear esomeprazole a bit faster and leave slightly less of it working. In practice, that might mean a little less heartburn or acid-reflux relief, not a dangerous reaction.

I want to be honest about the strength here: this is based only on test-tube and animal research, and it has never been reported in actual people. So I wouldn't lose sleep over it. Still, if your reflux symptoms creep back after starting lentinan, mention it to your pharmacist or doctor before changing anything.
