Fentanyl citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the honest picture with shiitake-derived lentinan and fentanyl. The concern on file is that lentinan might speed up an enzyme called CYP2D6, which could lower levels of drugs that depend on it. But this is only seen in lab and animal research, not in real people, so I'd file it under 'theoretical' rather than something that's actually been observed.

One thing worth knowing: fentanyl is mainly cleared by a different enzyme (CYP3A4), not CYP2D6, so this particular interaction likely has little real effect on it. Even so, because fentanyl is a powerful opioid where small shifts matter for breathing and sedation, please run any lentinan product past your pharmacist or doctor before combining them.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for fentanyl. Your liver uses an enzyme called CYP3A4 to break this opioid down. Lab studies hint that lentinan might rev up that enzyme a bit, which in theory could clear fentanyl out of your system faster and leave less of it working. For a strong pain medicine, that could mean your pain relief feels weaker or doesn't last as long as expected.

I want to keep this in perspective. This is only seen in test tube and animal research, never reported in real people, so it may not matter at all in practice. Still, fentanyl is a powerful drug with little room for error, so please loop in your pharmacist or doctor before adding lentinan rather than adjusting anything yourself.
