Why this applies to Efavirenz

Efavirenz falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with efavirenz (Sustiva). Your liver uses an enzyme called CYP3A4 to help process this HIV medicine. Lab and animal studies hint that lentinan may rev up that enzyme a bit, which could in theory clear efavirenz from your body a little faster and lower its level in your blood.

Why does that matter? With HIV treatment, keeping drug levels steady is important. If a level drops too low, the virus has more room to grow and can even develop resistance over time. That said, this effect has only been seen in the lab, never confirmed in people, so it's far from certain. Don't change anything on your own. Just loop in your pharmacist or HIV provider before adding lentinan so they can keep an eye on things.

What Is Efavirenz Used For?

Brand name: Sustiva

Efavirenz is used to treat HIV-1 (human immunodeficiency virus type 1) infection. It is always used together with other antiretroviral medicines — never on its own.

It is approved for adults and for children who are at least 3 months old and weigh at least 3.5 kg (about 7.7 lbs). Both the capsule and tablet forms are approved for this use.
