Etravirine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the idea behind this pairing. Your body partly uses an enzyme called CYP2C19 to break down etravirine (Intelence). Lab research hints that lentinan, the shiitake mushroom beta-glucan, might speed that enzyme up. If that happened in real life, your body could clear etravirine a bit faster, leaving lower levels in your blood. For an HIV medicine, that's the part worth respecting, because lower drug levels can mean less control of the virus and, over time, a risk of resistance.

That said, this is only test-tube and animal data, not something seen in actual patients. So I wouldn't panic. Etravirine isn't a prodrug, so this points to weaker effect rather than more side effects. Before adding any lentinan supplement, please run it by your HIV provider or pharmacist first.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for etravirine (Intelence): your liver uses an enzyme called CYP3A4 to help process this HIV medicine. In test tube studies, lentinan seemed to speed that enzyme up a bit. If that happened in real people, your body could clear etravirine a little faster, which in theory could lower its levels and make it less effective at keeping HIV under control. That matters, because steady drug levels are what keep the virus suppressed.

I want to be straight with you though: this is only lab-based evidence, and it has never been seen in actual patients. So it's a maybe, not a sure thing. Still, with HIV meds I lean cautious. Please check with your pharmacist or doctor before adding lentinan, and don't change your etravirine on your own.

What Is Etravirine Used For?

Brand name: Intelence

Etravirine (brand name Intelence) is used to treat HIV-1 infection in people who have already received other antiretroviral (HIV) treatments — meaning it is meant for patients whose HIV has had a chance to develop resistance to earlier therapies. It is always used as part of a combination regimen with other HIV medications, never on its own.

Both the brand Intelence and generic etravirine tablets are approved for adults and for children ages 2 to under 18 who weigh at least 10 kg (about 22 pounds) and who have already been treated with other HIV drugs.
