Ketoconazole falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to keep in mind with this combination: oral ketoconazole is already known for being tough on the liver. It's one of the reasons doctors reserve it for fungal infections when other options won't work. Padang cassia, a cassia-type cinnamon, naturally contains coumarin, which in large amounts has been linked to liver stress that usually clears up once the person stops taking it.

So the worry isn't a dramatic chemical clash. It's simply piling one liver burden on top of another. Watch for signs like yellowing of the eyes or skin, dark urine, belly pain, nausea, or unusual tiredness. This is mostly theoretical and based on pharmacology, and a sprinkle of cinnamon on your oatmeal is unlikely to matter. Concentrated cinnamon capsules are the real concern, so check with your pharmacist or doctor first.

Ketoconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for ketoconazole. Your body partly clears this antifungal using a liver enzyme called CYP3A4. In lab dishes, an extract of Padang cassia slowed that same enzyme down. In theory, if the enzyme works slower, ketoconazole could linger longer and build up, which may bump up its effects and side effects. With ketoconazole that could mean things like nausea, stomach upset, headache, or strain on the liver (and ketoconazole is already known for liver concerns).

The honest truth: this is only a test-tube finding, not something proven in people, so I wouldn't lose sleep over it. Cinnamon sprinkled on food is unlikely to matter. Concentrated cinnamon supplements are a different story, so check with your pharmacist or doctor before combining them.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
