Padang Cassia and Testosterone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Padang Cassia interacts with whole categories of medication. Testosterone falls into 2 of those categories, so it can interact with Padang Cassia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

HEPATOTOXIC DRUGS

1 of 2

Why this applies to Testosterone

Testosterone falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the connection worth knowing. Padang cassia (a type of cassia cinnamon) naturally contains coumarin, a compound that, in very large or concentrated supplement doses, can stress the liver. Testosterone itself, especially oral or injectable forms, can occasionally affect liver enzymes too. Stacking two things that may tax the liver is the concern here.

To be clear, this is a theoretical worry based on how these compounds behave, not strong proof of harm. The amount of cinnamon you sprinkle on food or in coffee is very unlikely to matter. The caution applies more to concentrated cinnamon capsules taken daily, or if you already have liver problems. Before adding a cinnamon supplement, check with your pharmacist or doctor, who may simply keep an eye on your liver labs.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Testosterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body partly uses a liver enzyme called CYP3A4 to break down testosterone. Padang cassia (a type of cassia cinnamon) has been shown in test tube studies to slow that enzyme down. In theory, if the enzyme works slower, a bit more testosterone could hang around, which might nudge up its effects and side effects, things like acne, oily skin, mood changes, or fluid retention.

I want to be honest about how soft this evidence is. It comes only from lab work, not from people, so we really don't know if it matters in real life. Sprinkling cinnamon on your oatmeal is almost certainly fine. If you're thinking about concentrated cinnamon capsules, just run it by your pharmacist or doctor first.

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Testosterone page

“The supplement list for testosterone therapy (Androgel, Testim, Nebido, and others) deserves real attention, mainly because five of its 170 entries are rated major and four of those are everyday items: grapefruit, St. John's wort, and alcohol in the form of beer or wine.”

About Padang Cassia

What is Padang Cassia?

  • Adding flavor to food
  • Digestive upset
  • Blood sugar support
  • Antimicrobial purposes
  • Antioxidant support

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Testosterone

What Is Testosterone Used For?

Brand names: Intrinsa, Testim, Androgel, Andropatch, Testogel, Nebido, Delatestryl, Natesto, Vogelxo

Testosterone replacement therapy is used in adult males whose bodies do not produce enough of their own testosterone. This includes primary hypogonadism — where the testes themselves fail due to causes such as Klinefelter's syndrome, injury, chemotherapy, or surgical removal — and hypogonadotropic hypogonadism, where the brain's signaling system (the hypothalamus or pituitary gland) fails to trigger testosterone production.

Testopel (subcutaneous pellets) may also be used to stimulate puberty in carefully selected males with clearly delayed puberty. The safety and efficacy of testosterone for age-related (late-onset) hypogonadism have not been established for most products, including AndroGel, Testim, Jatenzo, Kyzatrex, Natesto, Depo-Testosterone, Testosterone Cypionate, and Xyosted.
