Tacrolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the short version: your body clears tacrolimus mainly through a liver enzyme called CYP3A4. In lab dishes, a concentrated extract of Padang cassia (a cassia-type cinnamon) appeared to slow that enzyme down. If that were to happen in a person, tacrolimus could clear more slowly and build up in your blood, raising the odds of side effects like kidney strain, high blood pressure, shaky hands, headaches, or higher blood sugar and potassium.

I want to keep this in perspective. This is theoretical, based only on test-tube work, not on people taking the two together. Still, tacrolimus is a fussy drug where small changes really matter. Sprinkling cinnamon on your food is fine, but please check with your transplant team before starting any concentrated cinnamon supplement.
