There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to know about pairing concentrated Padang cassia with acarbose. This isn't a case of the cinnamon changing how acarbose works in your gut, where it slows sugar breakdown after meals. Instead, the concern is about your liver. Cassia cinnamon naturally contains coumarin, and in large supplement doses coumarin has been linked to liver irritation. Acarbose itself can occasionally raise liver enzymes, so stacking two things that can stress the liver is the worry here.

To be fair, this is mostly a theoretical concern based on how these compounds behave, not strong human proof. Sprinkling cinnamon on your food is not the issue. If you're thinking about daily cinnamon capsules, please run it by your pharmacist or doctor first, especially if you have any liver problems.

Some clinical research shows that Padang cassia can reduce blood glucose levels in healthy patients and in patients with diabetes or impaired glucose tolerance (prediabetes). Theoretically, Padang cassia might have additive effects in patients treated with antidiabetes drugs. Dose adjustments to diabetes medications may be necessary.

Here's the gist: some lower-quality studies suggest Padang cassia (a cassia-type cinnamon) can nudge blood sugar down a bit on its own. Acarbose already works to lower your after-meal blood sugar by slowing how fast carbs are digested. Stack the two together and the effects could add up, so your numbers might drift lower than expected. This isn't about the cinnamon changing how your body processes acarbose, it's simply two blood-sugar-lowering effects in the same direction.

In practice, watch for signs of low blood sugar like shakiness, sweating, or feeling lightheaded. Sprinkling cinnamon on your oatmeal is very unlikely to matter, but concentrated cinnamon capsules are a different story. Before starting a supplement, check in with your pharmacist or doctor so they can keep an eye on your readings.
