In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the short version: your liver uses an enzyme called CYP3A4 to help clear prednisolone from your body. Lab tests suggest a concentrated extract of Padang cassia may slow that enzyme down a bit. In theory, that could let prednisolone linger a little longer, which might nudge up its effects and side effects, things like fluid retention, higher blood sugar, mood or sleep changes, stomach upset, or easier bruising.

I want to keep this in perspective, though. This is only test-tube evidence, not something proven in people, so the real-world impact is likely small. Sprinkling cinnamon on your oatmeal is no worry at all. If you're thinking about concentrated cinnamon capsules alongside prednisolone, just run it by your pharmacist or doctor first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
