Clindamycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the simple version. Your liver uses an enzyme called CYP3A4 to help break down and clear clindamycin. In lab dish studies, concentrated Padang cassia (a cassia-type cinnamon) appears to slow that enzyme down. If that happened in the body, clindamycin could clear a little more slowly and build up, which could mean a bit more of its usual side effects like upset stomach, nausea, or diarrhea.

I want to be honest about how thin this evidence is. It comes only from test-tube work, not from people, so it's theoretical. Sprinkling cinnamon on your oatmeal almost certainly won't matter. The bigger question is concentrated cinnamon capsules. If you take those, just mention it to your pharmacist or doctor before starting clindamycin.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. Clindamycin is prescribed for serious infections caused by certain bacteria — including anaerobic bacteria (bacteria that thrive without oxygen), streptococci, staphylococci, and pneumococci. It's often chosen when penicillin-type antibiotics can't be used, such as in people with a penicillin allergy.

It can treat infections of the lungs, skin and soft tissue, bloodstream, abdomen, and female reproductive organs. Because it carries real risks, doctors reserve it for infections where safer antibiotics aren't appropriate.
