Padang Cassia and Levonorgestrel

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Padang Cassia interacts with whole categories of medication. Levonorgestrel falls into 2 of those categories, so it can interact with Padang Cassia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

HEPATOTOXIC DRUGS

1 of 2

Why this applies to Levonorgestrel

Levonorgestrel falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: this isn't about cinnamon weakening or strengthening your levonorgestrel (like Plan B or Mirena). It's about your liver. Padang cassia, a type of cassia cinnamon, naturally contains a compound called coumarin, which in very large or concentrated supplement doses has been linked to liver irritation that usually clears up once you stop. Since some medicines can also stress the liver, the theory is that piling them together might add up in sensitive people.

Honestly, this concern is mostly theoretical and based on high-dose coumarin, not real reports with levonorgestrel. Sprinkling cinnamon on your food is not a worry. If you regularly take strong cinnamon capsules or have any liver trouble, just mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Levonorgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for the morning-after pill. Your body partly clears levonorgestrel using a liver enzyme called CYP3A4. In a lab dish, an extract of Padang cassia slowed that enzyme down. If that held true in people, your levonorgestrel might be cleared a little more slowly and hang around longer. Levonorgestrel is active as-is (not a prodrug), so blocking that enzyme would tend to raise its levels, possibly bumping up side effects like nausea, headache, breast tenderness, or some menstrual spotting.

Honestly, though, this is only a test-tube finding, not something seen in real patients. Sprinkling cinnamon on your food won't matter here. If you take concentrated cinnamon capsules, just mention it to your pharmacist or doctor.

About Padang Cassia

What is Padang Cassia?

  • Adding flavor to food
  • Digestive upset
  • Blood sugar support
  • Antimicrobial purposes
  • Antioxidant support

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.
