Here's the thing to keep in mind: rifampin can be tough on your liver on its own, and it's one of the medications we watch closely for liver irritation during TB treatment. Padang cassia (a cassia-type cinnamon) naturally contains coumarin, which in large amounts has been linked to liver stress. So the worry is simply adding two liver burdens together.
This concern is theoretical, based on how these things work rather than strong human studies, so I don't want to alarm you. The cinnamon you sprinkle on oatmeal or toast is not the issue. Concentrated cinnamon capsules taken daily are where it could matter. Watch for warning signs like yellowing skin or eyes, dark urine, nausea, or belly pain, and check with your doctor or pharmacist before adding a cinnamon supplement.

Here's the honest picture with rifampin and Padang cassia (a cassia cinnamon). The concern on file is that, in a test-tube study, a cinnamon extract slowed down an enzyme called CYP3A4, which helps clear many drugs. In theory, slowing that enzyme could let a drug build up a bit, leading to stronger effects or more side effects like nausea, stomach upset, or liver irritation.
But this is only lab data, not human studies, so it's a weak, theoretical signal. On top of that, rifampin is actually one of the strongest enzyme boosters your body has, so a little cinnamon is unlikely to meaningfully change how it works. Sprinkling cinnamon on food is no worry. Before taking concentrated cinnamon capsules with rifampin, just check with your pharmacist or doctor.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Rifampin Used For?
Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor
Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.
Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
