Fermented Milk and Enalapril Maleate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Enalapril Maleate

Enalapril Maleate falls under ANTIHYPERTENSIVE DRUGS — the group Fermented Milk interacts with.

Theoretically, fermented milk might increase the risk of hypotension when taken with antihypertensive drugs.
Clinical research shows that drinking fermented milk can decrease systolic, but not diastolic, blood pressure in some patients.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: enalapril (Vasotec, Epaned) already lowers your blood pressure, and some research suggests that regularly drinking fermented milk, like kefir or certain probiotic yogurts, can nudge your top number (systolic pressure) down a little too. Put the two together and, in theory, your blood pressure could dip slightly lower than expected.

This is a mild, mostly theoretical concern based on how things work, not a dangerous combo. The amount of pressure drop from food-style servings is usually small. If your pressure ran too low, you might feel dizzy, lightheaded, or tired, especially when standing up quickly. Enjoying fermented milk is generally fine, but if you notice those symptoms, check in with your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Enalapril Maleate page

“Potassium is the supplement I worry about first with enalapril maleate (Vasotec, Epaned). This ACE inhibitor, used for high blood pressure and heart failure, already makes the body hold onto potassium, so adding more can raise blood levels too far, a problem called hyperkalemia.”
