Fermented Milk and Tigecycline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Tigecycline

Tigecycline falls under ANTIBIOTIC DRUGS — the group Fermented Milk interacts with.

Theoretically, antibiotic drugs might decrease the effectiveness of fermented milk.
Some fermented milk preparations contain live and active organisms, including lactobacilli and bifidobacteria strains. Simultaneously taking antibiotics might kill a significant number of these organisms. Tell patients to separate administration of antibiotics and fermented milk preparations by at least 2 hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: this interaction runs in one direction, and it's the fermented milk that loses out, not your antibiotic. Tigecycline is given as an IV in the hospital, so it's working throughout your body to fight infection. Since it kills bacteria, it can also wipe out the good live probiotic cultures (like lactobacilli and bifidobacteria) found in things like yogurt or kefir.

This won't make tigecycline weaker or more dangerous, and it won't add side effects. It just means the probiotic benefit of the fermented milk may be reduced while you're on the antibiotic. This is a theoretical concern based on how the two work, not strong proof. If you want the probiotic perk, spacing them a couple hours apart is a reasonable idea. Ask your pharmacist if you have questions.

From our pharmacist’s overview on the Tigecycline page

“Tigecycline (Tygacil) is an intravenous antibiotic usually reserved for serious or resistant infections, and its supplement list deserves real attention. About Tigecycline Injection

What Is Tigecycline Injection Used For?

Brand name: Tygacil

Tygacil (tigecycline for injection) is approved for adults 18 and older to treat three types of serious bacterial infections: complicated skin and skin structure infections, complicated intra-abdominal (stomach and intestinal) infections, and community-acquired bacterial pneumonia (pneumonia caught outside a hospital).

It is not approved for diabetic foot infections or for pneumonia caught in a hospital setting, including ventilator-associated pneumonia — clinical trials showed it did not work well enough for those uses and was associated with higher mortality in patients with ventilator-associated pneumonia.
