Acetaminophen, Pamabrom, Pyrilamine falls under DIURETIC DRUGS — the group Figwort interacts with.

Theoretically, overuse of figwort might compound diuretic-induced potassium loss. There is some concern that people taking figwort along with potassium depleting diuretics might have an increased risk for hypokalemia. Initiation of potassium supplementation or an increase in potassium supplement dose may be necessary for some patients. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.

Here's the connection to keep in mind. The product you asked about (Midol or Pamprin types) includes pamabrom, which is a mild diuretic, or water pill. Figwort, in concentrated supplement form, is thought to nudge the body toward losing more potassium too. Stack those two together and, in theory, your potassium could dip a little lower than usual. Low potassium can show up as muscle cramps, weakness, or feeling unusually tired.

I want to be honest about how shaky this concern is. It's based on how these compounds behave on paper, not on real patient studies, and pamabrom is a fairly gentle diuretic used for short-term bloating. For occasional menstrual relief, this is unlikely to cause trouble. Still, if you use figwort regularly or have a heart condition, run it by your pharmacist or doctor first.
