Hydralazine, Hydrochlorothiazide, Reserpine falls under DIURETIC DRUGS — the group Figwort interacts with.

Theoretically, overuse of figwort might compound diuretic-induced potassium loss. There is some concern that people taking figwort along with potassium depleting diuretics might have an increased risk for hypokalemia. Initiation of potassium supplementation or an increase in potassium supplement dose may be necessary for some patients. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.

The product you're asking about combines three blood pressure medicines, and one of them, hydrochlorothiazide, is a thiazide diuretic (water pill) that can lower your body's potassium over time. The concern here is fairly simple: figwort has been thought to have its own mild diuretic-like effect, so taking a lot of it on top of this combo could, in theory, push your potassium down a bit further than the medicine alone would.

If potassium drops too low, you might notice muscle cramps, weakness, tiredness, or an irregular heartbeat. I'll be honest though, this is a theoretical worry based on how the plant might behave, not on real studies in people, so I wouldn't panic. Occasional or topical figwort is unlikely to matter much. If you take it regularly, just mention it to your pharmacist or doctor so they can check your potassium.
