Figwort and Chlorothiazide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

DIURETIC DRUGS

Why this applies to Chlorothiazide

Chlorothiazide falls under DIURETIC DRUGS — the group Figwort interacts with.

Theoretically, overuse of figwort might compound diuretic-induced potassium loss. There is some concern that people taking figwort along with potassium depleting diuretics might have an increased risk for hypokalemia. Initiation of potassium supplementation or an increase in potassium supplement dose may be necessary for some patients. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: chlorothiazide (Diuril) already nudges your kidneys to flush out extra water and salt, and along the way it can lower your potassium. The concern with figwort is that it may have a mild water-pill effect of its own, so stacking the two could theoretically push your potassium down a little further than the medication alone would.

I want to be honest about the evidence here. This is a theoretical caution based on how the plant behaves, not something proven in real patients. Still, low potassium can cause muscle cramps, weakness, tiredness, or an irregular heartbeat, so it's worth respecting. If you'd like to try figwort, check with your pharmacist or doctor first. They may simply want to keep an eye on your potassium level.

About Figwort

What is Figwort?

  • Skin conditions like eczema and psoriasis
  • Minor wounds and rashes
  • Swelling and inflammation
  • Mild constipation
  • Traditional 'detox' or lymphatic support

Figwort is a traditional herb used mainly for skin problems and minor swelling, but solid human studies are lacking, so its benefits remain unproven. It is generally used short-term and topically, and people with heart conditions, pregnant women, and nursing mothers should avoid it until more is known.

About Chlorothiazide

What Is Chlorothiazide Used For?

Brand name: Diuril

Chlorothiazide is used to reduce fluid buildup (edema) linked to conditions such as heart failure, liver cirrhosis (hepatic cirrhosis), and certain kidney disorders — including nephrotic syndrome, acute glomerulonephritis, and chronic renal failure. It is also used when edema is caused by corticosteroid or estrogen therapy.

The oral suspension (Diuril) is additionally approved to treat high blood pressure, either on its own or alongside other blood pressure medications. The intravenous forms — including Sodium Diuril, Chlorothiazide Sodium, and Chlorothiazide injection products — are reserved for treating edema in patients who cannot take oral medication or in emergency situations; these intravenous products are not approved for high blood pressure management.
