Why this applies to Avapritinib

Avapritinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the short version: your body relies on a liver and gut enzyme called CYP3A4 to break down avapritinib (Ayvakit). Goldenseal tends to slow that enzyme down. When the enzyme works slower, avapritinib can hang around longer and build up to higher levels than intended.

With this cancer medication, higher levels can mean more of its known side effects, things like swelling around the eyes or in the legs, fatigue, nausea, low blood counts, dizziness, or changes in memory and mood. Because avapritinib is a serious drug with a narrow margin for error, even a modest bump matters. The evidence here is real but moderate, so I wouldn't panic. Please don't start or stop anything on your own. Just check with your oncology team or pharmacist before using goldenseal.

Goldenseal is a popular North American herb that contains berberine, a compound studied for antimicrobial effects. However, strong human evidence for its many traditional uses is largely lacking, and it should be avoided during pregnancy and breastfeeding. Talk to a pharmacist or doctor before using it.

About Avapritinib

What Is Avapritinib Used For?

Brand name: Ayvakit

Ayvakit (avapritinib) is used to treat adults with unresectable (cannot be surgically removed) or metastatic (spread to other parts of the body) gastrointestinal stromal tumor (GIST) — but only in patients whose tumor has a specific genetic change called a PDGFRA exon 18 mutation, including the D842V mutation. A test is needed to confirm this mutation before starting treatment.

Ayvakit is also used to treat advanced systemic mastocytosis (AdvSM) — a serious condition where too many mast cells build up in the body — including its subtypes: aggressive systemic mastocytosis (ASM), systemic mastocytosis with an associated blood cancer (SM-AHN), and mast cell leukemia (MCL). It is also approved for indolent systemic mastocytosis (ISM), a less aggressive form. Ayvakit is not recommended for patients with AdvSM or ISM who have very low platelet counts (below 50 × 10⁹/L).
