Why this applies to Axitinib

Axitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist with goldenseal and Inlyta (axitinib). Your body relies on a liver enzyme called CYP3A4 to break axitinib down and clear it out. Goldenseal tends to slow that enzyme down, so axitinib may not get cleared as quickly. That can let levels of the drug creep up higher than your oncologist planned.

With axitinib, more drug in your system can mean more of its usual side effects, like high blood pressure, fatigue, diarrhea, nausea, or hand-foot skin reactions. This is a cancer medicine with a fairly tight dosing window, so even modest changes are worth taking seriously. The evidence here is moderate, not ironclad, but I'd play it safe. Please don't start or stop anything on your own. Check with your oncology team or pharmacist before combining these.

What Is Axitinib Used For?

Brand name: Inlyta

Axitinib (brand name Inlyta) is used to treat advanced renal cell carcinoma (RCC), a type of kidney cancer. Inlyta can be used as a first-line treatment — meaning the very first treatment you receive — when combined with either avelumab or pembrolizumab, two other cancer medicines.

Inlyta is also approved as a single agent (on its own) for patients with advanced RCC who have already tried one other systemic (body-wide) cancer therapy that did not work or stopped working.
