Why this applies to Cyclophosphamide

Cyclophosphamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's a wrinkle worth knowing about: cyclophosphamide is a prodrug. It arrives in your body inactive and has to be switched on by liver enzymes, including CYP3A4, into its cancer-fighting form. Goldenseal tends to block CYP3A4. So unlike most drugs (which build up when an enzyme is slowed), goldenseal could mean less of the active medicine gets made, potentially weakening the treatment you really need to work.

The picture isn't perfectly clean, since other enzymes also activate this drug, but with a serious chemotherapy I'd rather be cautious. Food-amount goldenseal is one thing; concentrated supplement capsules are another. Please don't start, stop, or adjust anything on your own. Talk with your oncology team or pharmacist before adding goldenseal.

About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
