Leniolisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the short version for anyone taking Joenja (leniolisib): your body relies on a liver and gut enzyme called CYP3A4 to clear this medication. Goldenseal, thanks to compounds like berberine and hydrastine, tends to slow that enzyme down. When the cleanup crew works slower, more leniolisib can linger in your bloodstream, which may nudge up its effects and the chance of side effects like diarrhea, headache, or changes in your blood counts.

Leniolisib is an active drug on its own, not a prodrug, so this pushes in the direction of more drug exposure, not less. The evidence here is moderate and comes mostly from other CYP3A4 drugs, not leniolisib specifically. Please don't stop either one on your own. Just run it by your pharmacist or doctor first.

What Is Leniolisib Used For?

Brand name: Joenja

Joenja (leniolisib) is approved to treat activated phosphoinositide 3-kinase delta syndrome, commonly called APDS — a rare genetic immune disorder in which certain immune cells (B and T cells) don't work properly. It is approved for adults and pediatric patients who are at least 12 years old.

APDS is caused by gene variants that make a specific enzyme (PI3K-delta) overactive, leading to immune system dysfunction. Joenja is the brand name for leniolisib in tablet form, taken by mouth.
