Why this applies to Lonafarnib

Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist: your body uses a liver enzyme called CYP2C9 to help clear Lonafarnib (Zokinvy). In lab dish studies, goldenseal can mildly slow down that same enzyme. If that happened in real life, your Lonafarnib levels could creep up, which might mean more of its usual side effects, things like nausea, vomiting, diarrhea, poor appetite, tiredness, or changes in lab values your care team watches.

I want to be honest about the strength of this, though. This is only test-tube data, and it has not been shown to happen in people. So it's more of a caution than a proven problem. Because Lonafarnib is a serious specialty medicine, please loop in your doctor or pharmacist before adding goldenseal so they can guide you safely.

Lonafarnib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the thing about Zokinvy (lonafarnib): your body relies on a liver enzyme called CYP3A4 to break it down and clear it out. Goldenseal tends to slow that enzyme down. So if you pair the two, lonafarnib can build up in your bloodstream higher than intended.

Why does that matter? Lonafarnib already comes with side effects like nausea, vomiting, diarrhea, poor appetite, and tiredness, and higher levels could make these worse. This drug is also dosed very carefully and has known interactions with strong CYP3A blockers, so even modest changes can be a real concern. Please don't start or stop anything on your own. Just loop in your pharmacist or doctor before adding goldenseal so they can guide you safely.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
