Why this applies to Pimozide

Pimozide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the key thing about pimozide: your body clears it using a liver enzyme called CYP3A4. Goldenseal tends to slow that enzyme down. So if you take goldenseal alongside pimozide, the drug can build up in your blood higher than intended. Pimozide is active as-is (not a prodrug), so more of it means stronger effects and a greater chance of side effects.

What matters most here is pimozide's effect on the heart. Even at normal doses it can stretch out the heart's electrical rhythm (called QT prolongation), and higher levels raise the risk of dangerous, irregular heartbeats. You might also notice more drowsiness, stiffness, or restlessness. This is a real concern, so please don't add goldenseal without first checking with your pharmacist or doctor.

What Is Pimozide Used For?

Pimozide tablets are used to suppress the motor tics (involuntary movements) and phonic tics (involuntary sounds or vocalizations) that occur in Tourette's Disorder. It is reserved for patients whose daily life or development is severely affected by these tics.

Pimozide is not a first-choice treatment, and it is not meant for tics that are only mildly bothersome or cosmetic in nature. It should only be considered when standard treatments have not worked well enough.
