Why this applies to Treprostinil

Treprostinil falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the situation with these two. Treprostinil (Orenitram) is used for pulmonary arterial hypertension, and one of its built-in effects is widening blood vessels, which can lower your blood pressure. Goldenseal contains berberine, a plant compound that may also nudge blood pressure downward. Put them together and, in theory, you could see a bit more of a drop than usual.

I'll be honest with you though, this concern is mostly theoretical. It's based on how these things work and a little animal research, not on real reports of goldenseal plus treprostinil causing problems. Still, with treprostinil it's worth watching for dizziness, lightheadedness, headache, flushing, or feeling faint, especially when you stand up. Please check with your pharmacist or doctor before adding goldenseal rather than starting it on your own.

Theoretically, goldenseal might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Goldenseal contains berberine. In vitro and animal research shows that berberine can inhibit platelet aggregation. However, this effect has not been reported in humans.

Here's the connection worth knowing about. Treprostinil (Orenitram) is used to treat pulmonary arterial hypertension, and one of the things it does is relax blood vessels and gently discourage platelets from clumping together. Goldenseal contains berberine, which in lab dishes and animal studies has also nudged platelets toward sticking together less. Stack those two effects and, in theory, you could tip a little further toward easy bruising or bleeding.

I want to be upfront: this is a theoretical concern based on test-tube and animal data, and it has not actually been seen in people. So please don't panic. Still, since both can affect bleeding, it's smart to mention any goldenseal supplement to your pharmacist or doctor before combining them, and to watch for unusual bruising, nosebleeds, or bleeding gums.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
