There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the gist with alogliptin (Nesina) and gotu kola. This isn't about gotu kola changing how your diabetes pill works or your blood sugar numbers. Instead, the concern is the liver. There are a handful of case reports where gotu kola was linked to liver irritation, and alogliptin, like many medicines, can occasionally stress the liver too. The worry is that stacking the two might add up.

I'll be honest, the evidence here is thin and mostly anecdotal, so I wouldn't panic. Still, it's worth watching for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Before adding gotu kola, check with your pharmacist or doctor, especially if you've ever had liver trouble.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
