CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Bellafoline, Caffeine, Ergotamine Tartrate, Pentobarbital falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill (Cafergot PB) is mostly used for migraines, and the part that matters here is the caffeine. Caffeine is cleared from your body by an enzyme called CYP1A2. The thinking is that grape juice may gently speed that enzyme up, which could lower caffeine levels and make the caffeine portion of your pill work a bit less. In plain terms, you might notice slightly less of the usual caffeine kick.
Honestly, this is a theoretical concern based on one small study, so I would not lose sleep over it. A few grapes or normal juice with a meal is very unlikely to change how your migraine pill works. Concentrated grape seed supplements are less studied, so check with your pharmacist or doctor before starting one.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Bellafoline, Caffeine, Ergotamine Tartrate, Pentobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

This combination pill (Cafergot PB) pairs ergotamine for migraines with caffeine, belladonna alkaloids, and a barbiturate. The piece that matters most here is ergotamine, which the liver clears using an enzyme called CYP3A4. Grape seed extract has shown mixed signals in lab and animal studies, sometimes slowing this enzyme and sometimes speeding it up, so we honestly can't say which direction it would go in people.
If grape seed slowed the enzyme, ergotamine could build up, and that's worth watching because too much ergotamine can cause cold or numb fingers and toes, nausea, or tingling. Keep in mind this is only theoretical right now and hasn't been seen in humans. Eating grapes is fine. Before adding a concentrated grape supplement, check with your pharmacist or doctor.
