Homatropine Methylbromide, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo. Hydrocodone is actually a prodrug in part. Your body uses an enzyme called CYP2D6 to convert some of it into hydromorphone, which gives a chunk of the pain-relieving punch. Grape seed extract has shown, in lab dish studies only, that it might slow that same CYP2D6 enzyme down.
If that slowdown happened in a real person, you'd expect a little less conversion to the stronger active form, so possibly slightly weaker pain or cough relief. Honestly though, this has never been seen in humans, so I wouldn't lose sleep over it. Eating grapes won't matter at all. If you're taking concentrated grape seed extract pills, just mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

This cough and cold combo pairs hydrocodone (an opioid) with homatropine. The hydrocodone part is processed in part by a liver enzyme called CYP3A4, and grape seed extract may nudge that enzyme. Here's the honest truth: the lab and animal studies disagree about whether grape extract speeds the enzyme up or slows it down, and nothing has actually been seen in real people. So this is mostly a theoretical concern.
If the enzyme were slowed, hydrocodone could build up and lead to more drowsiness, dizziness, constipation, or slowed breathing. If sped up, the medicine might not work as well. Eating grapes won't matter, but before starting a concentrated grape seed supplement, run it past your pharmacist or doctor.
