Here's the simple version: Roxithromycin is an antibiotic you swallow, and it has to be absorbed from your gut to fight the infection. Guar gum, especially in concentrated supplement form, turns into a thick gel in your stomach and intestines. That gel can trap part of the antibiotic and slow or reduce how much actually gets into your bloodstream.

The worry isn't extra side effects, it's the opposite: you might get less of the drug than you need, which could mean your infection isn't treated as well as it should be. The easy fix is timing. Take your roxithromycin first, then wait about 30 to 60 minutes before your guar gum. Food amounts of guar gum are unlikely to matter much, but check with your pharmacist or doctor to be sure.
