Here's the gist for Ramelteon (Rozerem): your liver uses an enzyme called CYP3A4 to break this sleep medication down. Lab studies hint that the active compounds in guggul (guggulsterones) may speed that enzyme up. If that happens in real people, your body could clear Ramelteon a little faster, which might leave you with a weaker effect. In plain terms, you may notice it doesn't help you fall asleep as well as it usually does.

I want to be honest about the strength of this: it's theoretical, based only on lab and animal work, not human studies, so it may not amount to much. Ramelteon isn't a prodrug, so this isn't a safety scare. Still, if your sleep gets worse after adding guggul, mention it to your pharmacist or doctor before changing anything.
