Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA) which are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the thing to understand about this pairing. Hemp agrimony contains natural compounds called pyrrolizidine alkaloids that can be hard on the liver. Your body uses an enzyme called CYP3A4 to process them, and unfortunately that processing can turn them into more harmful, liver-damaging forms. Givinostat is thought to gently speed up (induce) that same CYP3A4 enzyme, so in theory it could nudge more of those alkaloids toward the toxic version.

I want to be honest about the evidence here: this is a theoretical concern based on lab work, not real patient cases. But since hemp agrimony is already tough on the liver on its own, and most experts advise against swallowing it at all, I'd steer clear while you're on givinostat. Please check with your doctor or pharmacist before using any product containing it.
