Olutasidenib falls under HEPATOTOXIC DRUGS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA). Theoretically, using hemp agrimony with hepatotoxic drugs might have additive adverse effects on the liver. Some drugs that can adversely affect the liver include acetaminophen (Tylenol and others), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), fluconazole (Diflucan), itraconazole (Sporanox), erythromycin (Erythrocin, Ilosone, others), phenytoin (Dilantin), lovastatin (Mevacor), pravastatin (Pravachol), simvastatin (Zocor), and many others.

Here's the concern with pairing these two. Olutasidenib (Rezlidihia) is a cancer medicine for certain types of acute myeloid leukemia, and it already carries a known risk of liver injury. Your care team checks your liver enzymes regularly while you're on it for that very reason.

Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds that may be toxic to the liver. Stacking a liver-stressing herb on top of a drug that can already strain the liver could, in theory, add up to more liver damage. This worry is based on how these substances behave, not on actual studies in people, so it's theoretical. Still, given the stakes, I'd skip hemp agrimony here. Please loop in your oncology pharmacist or doctor before using any product that contains it.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA) which are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the key thing to understand with this pairing. Hemp agrimony naturally contains pyrrolizidine alkaloids, which can be hard on the liver. Those alkaloids get turned into their harmful forms by a liver enzyme called CYP3A4. Olutasidenib (Rezlidihia) speeds up that same enzyme, so in theory it could ramp up the conversion of those plant compounds into liver-damaging byproducts.

That matters extra here because olutasidenib itself can already raise liver enzymes and cause liver problems as a known side effect, so stacking another liver stressor on top isn't ideal. To be clear, this concern comes only from lab studies, not from people, so it's theoretical. Still, because of the liver risk, I'd skip hemp agrimony and check with your oncology team or pharmacist first.

Rezlidhia (olutasidenib) is used to treat adults with acute myeloid leukemia (AML) — a fast-moving blood cancer — when the disease has come back or stopped responding to previous treatment. It is only for patients whose leukemia cells carry a susceptible IDH1 mutation, which must be confirmed by an FDA-approved test before starting treatment.
