Vadadustat falls under HEPATOTOXIC DRUGS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA). Theoretically, using hemp agrimony with hepatotoxic drugs might have additive adverse effects on the liver. Some drugs that can adversely affect the liver include acetaminophen (Tylenol and others), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), fluconazole (Diflucan), itraconazole (Sporanox), erythromycin (Erythrocin, Ilosone, others), phenytoin (Dilantin), lovastatin (Mevacor), pravastatin (Pravachol), simvastatin (Zocor), and many others.

Here's the concern with pairing hemp agrimony and Vadadustat (Vafseo), your medicine for anemia related to chronic kidney disease. This isn't about one changing how the other is broken down. Instead, it's about doubling up on liver stress. Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds that may harm the liver, and Vadadustat itself can raise liver enzymes in some people. Using both could theoretically add up and be harder on your liver.

I'll be honest, this is a theoretical caution based on how these two behave, not on studies of people taking them together. Still, because of the liver risk from those alkaloids, I'd steer clear of hemp agrimony internally and check with your doctor or pharmacist before using any product that contains it.
