Acetaminophen, Aspirin, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the bigger picture with combo products like Excedrin (acetaminophen, aspirin, and caffeine). Lab studies suggest higenamine may slightly block a liver enzyme called CYP3A4, which could in theory let certain medications build up a bit. In practice, this matters very little for this particular pill, since acetaminophen, aspirin, and caffeine aren't mainly cleared by CYP3A4, and the effect was only seen in a test tube (about a 21% drop), never confirmed in people.

The more real-world concern is overlap: both higenamine and the caffeine in Excedrin are stimulants. Stacking them can leave you jittery, with a racing heart, higher blood pressure, or trouble sleeping. If you take any of these regularly, please run it by your pharmacist or doctor first.

Here's the thing with this combo product (think Excedrin Migraine): the aspirin part already thins your blood a bit by making platelets less sticky. Higenamine, in animal studies, seems to do something similar, keeping platelets from clumping together. Stack the two and, at least on paper, you could bruise more easily or bleed a little longer from a cut. With aspirin, that can also mean some extra stomach irritation.

I want to be honest about the strength of this, though. This warning is theoretical and comes from lab and animal work, not solid human studies, so it's a possibility, not a sure thing. Since higenamine has its own heart and blood pressure concerns, it's worth running this past your pharmacist or doctor before combining them.

Theoretically, higenamine might increase the risk of cardiovascular toxicity when taken with stimulant drugs.
Higenamine has stimulant effects due to agonist activity at beta2-adrenoreceptors. In cardiac muscle, higenamine appears to have a positive inotropic effect and increase heart rate. However, it does not appear to increase blood pressure.

Here's the thing to focus on with Excedrin: it's the caffeine in this combo that matters, not the acetaminophen or aspirin. Caffeine is a stimulant, and higenamine acts a bit like adrenaline on your heart, so stacking the two could add up. In theory, you might feel more of that revved-up jittery feeling, a faster or pounding heartbeat, or restlessness.

I'll be honest, this concern is mostly theoretical and based on how these compounds work, not strong human studies. An occasional Excedrin Migraine is unlikely to be a big deal. Still, since higenamine products are poorly regulated and can stress the heart, please run this by your pharmacist or doctor first, especially if you have any heart or blood pressure issues.
