CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Acetaminophen, Butalbital, Caffeine, Codeine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo product: the codeine part is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Higenamine has been shown in lab dish studies to block CYP2D6. If that blocking happened in real life, less codeine would convert to morphine, so you might get less pain relief from your Fioricet with Codeine, not more.
I'll be straight with you though: this is only seen in test tubes, never confirmed in people, so it's mostly theoretical. Higenamine carries its own heart and blood pressure concerns, which matters more here. Please loop in your pharmacist or doctor before mixing the two.

In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill has a few moving parts, and the one that matters most here is the butalbital (a sedative barbiturate), which is partly cleared by the CYP3A4 enzyme. In lab tests only, higenamine slowed that enzyme down a little (about 21%). If that held true in people, your body might clear butalbital a bit more slowly, which could mean more drowsiness, dizziness, or grogginess than usual.
The honest truth is that this is theoretical. It has only been seen in a test tube, never confirmed in real people, so I wouldn't lose sleep over it. The bigger concern is that higenamine is a heart and blood pressure stimulant, while this medicine contains caffeine and a sedative. Please run this pair by your pharmacist or doctor before mixing them.

Higenamine has stimulant effects due to agonist activity at beta2-adrenoreceptors. In cardiac muscle, higenamine appears to have a positive inotropic effect and increase heart rate. However, it does not appear to increase blood pressure.

The piece of this combo pill that matters here is the caffeine. Caffeine is its own stimulant, and higenamine acts like a mild adrenaline booster on the heart. Stack them together and you could feel more of those jittery, stimulant-type effects: a faster or pounding heartbeat, restlessness, or feeling wired. This isn't about how the medicine is broken down. It's simply two stimulants pushing in the same direction.
Honestly, this concern is theoretical, based on how higenamine behaves in the body rather than solid human studies. Interestingly, higenamine doesn't seem to raise blood pressure much. Still, the supplement is poorly regulated and labels are often wrong, so I'd play it safe. Before mixing the two, especially if you have any heart rhythm or heart issues, check with your pharmacist or doctor.
