Alogliptin, Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the gist for Oseni, which is a combo of alogliptin and pioglitazone used to manage type 2 diabetes. The pioglitazone part is handled partly by a liver enzyme called CYP3A4. In a lab dish, higenamine slowed that enzyme down by about 21%. If that happened in real people, pioglitazone could build up a little, which in theory might mean more of its effects, like fluid retention, swelling, weight gain, or a low blood sugar episode if it's paired with other diabetes meds.

That said, this is only test-tube data and has never been confirmed in humans, so I wouldn't lose sleep over it. My bigger worry is higenamine itself, since it can raise heart rate and blood pressure. Please run it by your pharmacist or doctor before combining the two.
