Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the idea behind this pairing: your body uses an enzyme called CYP3A4 to help clear erythromycin out of your system. In a lab dish, higenamine slowed that enzyme down by about 21 percent. If that held true in people, erythromycin could linger a bit longer and its effects, including side effects, could be stronger than expected.

With erythromycin, that mostly means more stomach upset, nausea, or cramping, and in rare cases changes to your heart's rhythm (a longer QT interval). I want to be honest, though: this is only a theoretical concern from test-tube work, not something seen in actual patients. Still, since higenamine itself can stir up your heart and blood pressure, I'd run this combination by your pharmacist or doctor before mixing them.
