Here's the simple version: hops may have a mild calming, sleepy effect, and Toviaz (fesoterodine) can already make some people feel a little drowsy or tired on its own. Put the two together and that sleepy feeling could add up, leaving you more groggy than expected, especially when you first start.

I want to be honest about how solid this is. The concern is theoretical, based mostly on animal studies and how hops is thought to work, not on strong human data, so for many folks it's a non-issue. Still, go easy if you're driving or doing anything that needs full focus, and check with your pharmacist or doctor before pairing them, particularly if you take other things that cause drowsiness.

Fesoterodine Fumarate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for Toviaz (fesoterodine): your liver uses an enzyme called CYP3A4 to help clear it from your body. Some lab and animal studies hint that certain hops compounds (lupulones) might gently speed up that enzyme, which could in theory lower your fesoterodine levels and make it a touch less effective for bladder control. The flip side, since fesoterodine relies on this pathway, would be slightly less relief from urgency, frequency, or leaks.

The honest part: this is theoretical and very weak. A human study using hops barely nudged a similar drug (about 8%), which isn't enough to matter for most people. I wouldn't lose sleep over it, but if you take hops regularly, just mention it to your pharmacist or doctor.

Fesoterodine treats overactive bladder (OAB) in adults. That means symptoms like urge urinary incontinence (leaking with a sudden urge), urgency, and frequency.

Some products are also approved for neurogenic detrusor overactivity (NDO) in children 6 years and older who weigh more than 25 kg. This use is listed for Toviaz and for one fesoterodine fumarate film-coated extended-release tablet label. The other fesoterodine fumarate extended-release tablet label covers adult OAB only.
