Why this applies to Midostaurin

Midostaurin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist: your body uses an enzyme called CYP3A4 to break down midostaurin (Rydapt), and there's a theory that hops might nudge that enzyme to work a little faster. In plain terms, speeding up that cleanup could in theory lower midostaurin levels and make it slightly less effective against your leukemia or mastocytosis. Midostaurin is an active drug, not a prodrug, so faster clearance means less drug working, not more.

The good news is this concern is weak. It comes mostly from lab and animal studies, and a human study using a similar CYP3A4 drug saw only a tiny 7.6% change that wasn't meaningful. Still, because this is a cancer therapy where dosing really matters, please run any hops supplement past your oncologist or pharmacist first.

What Is Midostaurin Used For?

Brand name: Rydapt

  • Midostaurin is used with other chemotherapy drugs to treat certain types of acute myeloid leukemia (AML; a type of cancer of the white blood cells). Midostaurin is also used to certain types of mastocytosis (a blood disorder in which there are too many mast cells [a certain kind of white blood cell]). Midostaurin is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps stop the spread of mast and cancer cells.
