Why this applies to Naldemedine Tosylate

Naldemedine Tosylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for Symproic (naldemedine), the medicine that eases constipation from opioid pain relievers. Your body uses an enzyme called CYP3A4 to clear naldemedine out, and hops might nudge that enzyme to work a little faster. If that happened, your system could break down naldemedine slightly quicker and you might notice it working a bit less well, meaning your constipation relief could dip.

That said, the evidence here is very thin. It comes mostly from lab and animal studies, and a small human study found only about an 8% change with a similar drug, which isn't enough to matter for most people. Naldemedine is not a prodrug, so it doesn't flip the usual logic. If you take both regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

What Is Naldemedine Used For?

Brand name: Symproic

Symproic (naldemedine) is used to treat opioid-induced constipation (OIC) — constipation caused by opioid pain medications — in adults with chronic non-cancer pain. This includes people living with chronic pain related to prior cancer or its treatment, as long as they do not need frequent opioid dose increases (such as weekly adjustments).

It is not a general constipation remedy. Symproic works specifically against the constipating effect of opioids and should be stopped if you stop taking your opioid pain medication.
