Why this applies to Netupitant, Palonosetron (Oral)

Netupitant, Palonosetron (Oral) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the short version for Akynzeo (netupitant/palonosetron): the netupitant part is cleared by a liver enzyme called CYP3A4. Lab and animal studies hint that certain hops compounds (lupulones) might speed that enzyme up a bit. In theory, that could lower netupitant levels and make it slightly less effective at preventing chemo-related nausea and vomiting.

In real life, this concern looks very small. A human study using hops only nudged a similar CYP3A4 drug down by about 8 percent, which isn't enough to matter. Still, because keeping your nausea fully controlled during chemo is so important, I'd mention any hops product (or sleep blends with hops and valerian) to your oncology team or pharmacist before adding it, so nothing gets in the way of your treatment.

What Is Netupitant and Palonosetron Used For?

Brand name: Akynzeo

Akynzeo capsules are used in adults to prevent both the short-term (acute) and longer-lasting (delayed) nausea and vomiting that can follow cancer chemotherapy — including, but not limited to, highly emetogenic (strongly nausea-causing) chemotherapy regimens. It is always taken together with the steroid dexamethasone.

Akynzeo capsules cover a broad range of chemotherapy regimens, including anthracycline plus cyclophosphamide-based regimens. Each ingredient targets a different stage: palonosetron addresses nausea and vomiting in the first 24 hours, while netupitant covers both the early and the delayed phase that can last several days after treatment.
