Why this applies to Polatuzumab Vedotin-Piiq

Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the short version: the worry on file is that hops might speed up a liver enzyme called CYP3A4, which could in theory lower levels of medicines that depend on it. But this concern comes only from lab and animal studies, and the one human study showed barely any change (about an 8% drop in a test drug, which is not meaningful). So I wouldn't lose sleep over it.

With Polatuzumab vedotin-piiq (Polivy), it's also worth knowing this is an infusion given in a cancer clinic, where your team closely watches your blood counts, nerve symptoms, and infection risk. That said, never add hops or any supplement during cancer treatment without first clearing it with your oncology team or pharmacist.

What Is Polatuzumab vedotin-piiq Injection Used For?

Brand name: Polivy

  • Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
