Why this applies to Vorapaxar

Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the quick story on combining hops with vorapaxar. Your liver uses an enzyme called CYP3A4 to clear vorapaxar, and lab and animal studies hint that hops might nudge that enzyme to work a little faster. If that happened to a meaningful degree, your body could clear vorapaxar slightly quicker, which in theory might make this blood-thinning medicine a touch less effective at preventing clots, heart attack, or stroke.

But the real-world evidence is reassuring. A human study using a similar 3A4-cleared drug found only about an 8% drop in levels, which isn't enough to matter. So this is a minor, mostly theoretical concern. Still, because vorapaxar protects your heart, it's worth a quick mention to your pharmacist or doctor before adding hops.

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.

Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
