Acetaminophen, Chlorpheniramine, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This Coricidin product is a combo of three things: acetaminophen (Tylenol), the antihistamine chlorpheniramine, and the cough suppressant dextromethorphan. The concern here is a lab finding only. In a test tube, horny goat weed slowed down a liver enzyme called CYP1A2. If that happened in real life, certain ingredients could clear a bit more slowly and hang around longer.
In practice, this might mean a little more drowsiness, dizziness, or grogginess than usual. The honest takeaway, though, is that this has never been shown in actual people, only in cells, so I wouldn't lose sleep over it. Still, if you take horny goat weed regularly, mention it to your pharmacist or doctor before reaching for this medicine.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo product (a Coricidin-type flu medicine) has three ingredients, and the one that leans on the CYP3A4 enzyme is dextromethorphan, the cough suppressant. Horny goat weed has only been shown to slow CYP3A4 in test-tube studies, never in actual people. If it did slow that enzyme in your body, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or that floaty, foggy feeling. The chlorpheniramine (an antihistamine) can add to that sleepiness too.
Honestly, this is a theoretical concern based on lab work, so I wouldn't lose sleep over it. Still, if you're using both, mention it to your pharmacist, and skip the herb if you feel unusually groggy.
