Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist for anagrelide (Agrylin). Your body relies on a liver enzyme called CYP1A2 to help break this medicine down. In lab dish studies, horny goat weed appears to slow that same enzyme. If that happened in people, anagrelide could clear more slowly and build up a bit, which may strengthen its effects and side effects. Think headache, a pounding or racing heartbeat, palpitations, dizziness, fluid retention, or low blood pressure.

I want to be straight with you: this is only a theoretical concern seen in test tubes, not something reported in actual patients, so the real-world risk is uncertain. Still, since anagrelide affects your platelets and heart, please run this by your pharmacist or doctor before combining the two.

In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the thing to keep in mind: anagrelide already lowers your platelet count and gently slows clotting, which is exactly its job for essential thrombocythemia or polycythemia vera. Horny goat weed may add to that effect. In lab and animal studies, a compound in this herb (icariin) kept platelets from clumping together. Stack that on top of anagrelide and, in theory, you could bruise or bleed more easily, see nosebleeds, or notice cuts taking longer to stop.

I want to be honest about the evidence, though. This has only been seen in test tubes and animals, never confirmed in people, so the real-world risk may be small. Still, since both work in the same direction, please run it by your pharmacist or doctor before combining them.

What Is Anagrelide Used For?

Brand name: Agrylin

Anagrelide — sold as Agrylin and as generic anagrelide capsules — is used to treat thrombocythemia secondary to myeloproliferative neoplasms. That means it's prescribed when a bone marrow disorder causes the body to produce too many platelets, raising the risk of dangerous blood clots.

By bringing platelet counts down to a safer range, anagrelide also helps reduce related symptoms, including bleeding and clotting events (thrombo-hemorrhagic events).
