Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your bendamustine (Bendeka). One of the liver enzymes that helps break this chemo drug down is called CYP1A2. In test-tube studies, horny goat weed appears to slow that enzyme. If that same thing happened in people, your body could clear bendamustine a little more slowly, letting it build up. With a strong cancer drug, that could mean heavier side effects, things like low blood counts, more infections, fatigue, nausea, or mouth sores.
That said, this is theoretical. The slowing was only seen in the lab, not in actual patients, so we honestly don't know if it matters in real life. Because bendamustine is serious chemo, please don't add horny goat weed without first clearing it with your oncologist or pharmacist.

What Is Bendamustine Injection Used For?

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.
For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
