Theoretically, horny goat weed might increase the risk of bleeding.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the quick picture. Cangrelor (Kengreal) is a strong IV antiplatelet drug given in the hospital, usually during a cardiac stent procedure, to keep your blood from clotting too easily. Horny goat weed isn't a prodrug situation here, so don't overthink the chemistry: in lab and animal studies, this herb also seems to make platelets less sticky. Stack that on top of cangrelor and you're nudging in the same direction, which theoretically means a little more bleeding risk.

I want to be honest about how thin this evidence is. The blood-thinning effect of horny goat weed has only been seen in test tubes and animals, never confirmed in people. Still, since cangrelor is given in a tightly monitored setting, mention any supplements you take to your care team so they have the full picture.
