Cevimeline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP3A4 to help clear cevimeline (Evoxac) out of your system. In lab dish studies, horny goat weed seems to slow that same enzyme down. If that held true in people, cevimeline could hang around longer and its effects could feel stronger. With this drug, that usually means more of its cholinergic side effects, things like extra sweating, watery eyes, nausea, an upset stomach, a runny nose, or feeling flushed.

The honest part: this is only a theoretical concern based on test-tube research, and it has not been shown in humans. So I wouldn't panic, but I would run it past your pharmacist or doctor before combining them, especially if you have heart issues.
